It is the mission of the Small Providers Committee to identify and be responsive to the unique business strategies and environments of the small provider. Identify resources and develop effective educational and communications tools to augment the individual organization expertise.

To download the file in zipped format, click on link above and under "Utilities/Miscellaneous" click on "Zip Code to Carrier Locality File." Then, click on "Zip Code to Locality File" again to start the download. Save the file to your desktop and then unzip the contents.
